Permit forms will be available after 9 am the next business day after completing this form during normal instructional days. If your test is scheduled during a break from school (Fall Break, Winter Break, Spring Break, etc), please request your form at least three days in advance. Staff may not be available to prepare your form during breaks, so requesting early ensures the form will be ready in time for your test.
